Filling Out Forms
Dr. Nelson has three forms you fill out when you come in, plus another form you fill out each time when you return.
Some patients think filling out the forms, especially on return visits, is a pain. Actually, this form is part of our Continuous Quality Improvement program. The form gives Dr. Nelson information about how things have changed, as well as give you a chance to rate our service, tell us how to improve, bring things to our attention. Nonetheless, some patients consider it a pain. Therefore, this cartoon seems appropriate.
