I appreciate your coming to see me and hope that I can help you with your hand problem. In order to make sure that I get the history of your problem correct, I ask that you fill out three pages of information. I have designed them myself to be as quick and painless as possible. I hate forms, too, but this information will help me to help understand your problem.

2. Print the forms and complete them at home. If your hand problem makes it difficult for you to fill in the forms, wait until you get to the office and ask my secretary to help you.

NOTE: You print the blank forms on your printer and fill them out with a pen or pencil. You do not fill them out online.

3. Bring the forms with you to your appointment. Do not email the forms to me. This web site is not equipped to receive the forms electronically.
